Required Immunizations to Attend School

Washington state law requires any child attending in-person or online school, preschool, or child care to show proof of the required vaccines for their age. All students must have a Washington State Certificate of Immunization Status (CIS) form on file. The CIS form must have the required vaccine dates listed and vaccination records must be provider verified. Please talk with your health care provider or school staff if you have questions about school immunization requirements.

How can I show proof of vaccination?

The Washington State Department of Health requires students to show proof of the above listed required vaccines for school attendance. All students must have a Washington State Certificate of Immunization Status (CIS) form on file. The CIS form must have the required vaccine dates listed and vaccination records must be provider verified.

Please complete, sign, and turn in to your school. Please follow the instructions on page two, for appropriate form completion. You may also be able to access your child's immunization records through the Washington State Immunization Information System.

Medically Verified Records

A completed Certificate of Immunization Status (CIS) signed by a health care provider.
- Find the CIS form by visiting https://www.doh.wa.gov/SCCI and clicking on “Certificate of Immunization Status.”
A CIS filled out by you or another parent/guardian with medical records attached.
A CIS printed by a health care provider or school from the Washington State Immunization Information System.
A CIS printed from MyIR which is a free Department of Health online tool that allows families to view and print their official immunization records themselves.

How can I access my student's immunization records?

If you are a Washington state resident, you can download a copy of your child’s immunization record on the WA State Immunization Record website.

Go to the WA State Immunization Record website
Register for an account and then log in
Go to “MyImmunizations” at the top of the page and then click the “Certificate of Immunization” button to download the pre-filled CIS and save to your computer

Where can I find more information about vaccines?

New Immunization Tracking System

Starting in 2023-24, Tacoma Public Schools will use a different system, the Washington State Immunization Information System (WAIIS), to update and track all immunization records for our students. The system is a secure, web-based tool for healthcare providers and schools. It connects people who receive, administer, record and order vaccines in Washington. The Washington State Department of Health administers the WAIIS system.

A student may only attend school or childcare if their immunizations are up to date and Tacoma Public Schools has received records verified and signed by their medical provider.

Option 1

Ask your healthcare provider to complete the Certification of Immunization Status form and share the completed form with the state. Most care providers will do this free of charge. TPS does not recommend paying your care provider for this service.

Option 2

Give Tacoma Public Schools permission to update your records by completing these steps:

Complete the Certification of Immunization Status form, including signing the consent box in the top left, and share with TPS.
Share your verified and signed immunization records with TPS
Send both documents to tps_immunizations@tacoma.k12.wa.us
